Intellectual and Developmental Disability Provider Services

Provider Services include programs, services and supports provided by PermiaCare to eligible individuals in the community.

These services are intended to provide individuals the skills and supports to be able to continue to live in the community and to function to their potential.

The services are described in the following sections. They are delivered within two major types of funding sources:

  • State-supported General Revenue Services
  • Medicaid Waiver Services

General Revenue Services Available Through the LIDDA

Individualized Skills and Socialization

Site-based, as well as community-based, programming that assists with acquiring, retaining, or improving self-help, socialization, and adaptive skills necessary to live successfully in the community and to participate in home and community life. 

Vocational Services

Training Services provided to an individual in an industrial enclave, a work crew, a sheltered workshop, or an affirmative industry, to enable the consumer to obtain employment.

Employment Assistance/Supported Employment

Services to assist an individual in finding individualized competitive employment in the community and to sustain their employment.

Respite

Planned or emergency short-term relief services provided to the individual’s unpaid caregiver when the caregiver is temporarily unavailable to provide support.

Community Support Services

Individualized activities that are consistent with the consumer’s person-directed plan and provided in the home or community locations. Supports included activities that foster the ability to perform daily living skills, activities to preserve the family unit, transportation between home and employment or day habilitation site or community activities.

Nursing

Treatment and monitoring of health care procedures prescribed by a physician or medical practitioner or required by standards of professional practice or state law to be performed by licensed nursing personnel.

Medicaid Waiver Services

PermiaCare is certified for the provision of two Medicaid Waiver programs: Home and Community-based Services (HCS) and Texas Home Living Services (TxHmL).

Home and Community-Based Services (HCS)

Services and support available in a person’s own home or family home, or in a small residential setting. Residential settings include supervised living, residential support, and host homes. Additional services include adaptive aids, specialized counseling, audiology, speech/language pathology, occupational therapy, physical therapy, dietary services, social work, behavioral supports, minor home modifications, dental treatment, nursing, respite, day habilitation, vocational training, supported home living, and supported employment.

    Texas Home Living Program (TxHmL)

    Services and supports, such as, community support, day habilitation, employment assistance, supported employment, respite, nursing, behavioral support, physical therapy, occupational therapy, audiology, speech/language pathology, behavior supports, dietary services, minor home modifications, adaptive aids, and dental treatment, for persons who live in their own home or their family’s home.
