PermiaCare accepts Medicare, Medicaid, and some private health insurance for services rendered throughout all of our locations.

Payment for Services

As a courtesy to our consumers, PermiaCare will file claims with your insurance coverage. It is ultimately your responsibility for the full and timely payment of your account.

Payment of your estimated consumer portion is expected at the time services are rendered. This includes amounts for co-pays, coinsurance, deductibles & past-due balances unless previous arrangements have been made with our office. Payment of your copayments, deductibles & co-insurance is part of your contract agreement with your insurance plan. PermiaCare will estimate your financial responsibility; it is your insurance company that will make the final determination regarding your eligibility and benefits.

For persons who are not eligible for health care coverage, you may qualify for financial assistance.  This qualification will be determined at your initial visit with PermiaCare and at regular intervals thereafter. Fees will be assessed on the basis of proof of the individual’s (or, in some cases, the family’s) complete income and household size relative to a discount scale based on the federal poverty income guidelines, with no one being denied services due to an inability to pay.

To qualify, it is very important that you bring with you: Proof of Insurance, Proof of Residence, and Proof of Family Income.
